During the year of 1992, The London Ambulance Service was an ambulance service that provided services to over six million. Out of their 318 emergency ambulances, over half the amount were available around the clock the service also offered over four hundred transport ambulances, a helicopter, and a motorcycle response unit in which their system was managed from the LAS Headquarters in Waterloo. There were up to 2500 calls received daily, over 50 percent of which were requests for emergency services from this ambulance service. It showed great appreciation to it’s staff and significantly amount of its budget it was specifically to emergency response. Studies show the LAS does not directly charge its patients for its services and currently the largest ambulance service in the world. During the 1980s, the LAS emergency dispatch was a manually system and consisted of three main tasks: taking calls, identifying resources, and resource mobilization. From a perspective of this system in 2006, in which computers dominate everyday life, a manual system that relies on people’s memories, paper, and human reasoning for optimal resource utilization seemed unmanageable. The system would offer more than simply assisting dispatchers; this completely computerized system would do nearly everything electronically. A person would perform a sequence of steps which included answering the phone, data entry incident information into a computer terminal, and respond if the system displayed exception messages resulting from no ambulances being available over 10 minutes. The system would then use this map data and the location and status details provided by the AVLS to find and dispatch the available ambulance closest to the incident area simanteoulsly this system would be implemented not incrementally.
Along with issues, including a project cancellation and re-design, a software system

The AVLS had lost track of the ambulances and their statuses in the system. It sent
multiple units to some locations and some receive no locations, the efficiency with which it
assigned vehicles to call locations were substandard. The system began to generate such a huge
amount of exception messages on the dispatchers’ terminals that calls were even lost. The
problem was doubled when people called back additional times because the ambulances never
shown up after incidents were entered into the system, which congested the system. On the
following day, the LAS switched back to a part-manual system, and shut
down the computer system completely, the system no longer worked by the eight day.
The computer system failure affected many people and because of the large area
LAS serviced it could have avoid over 46 deaths if the request of service was fulfilled.
At the time the system went live, no load –tests had been run and there were 81 known issues. There were no provisions for a backup system. During the gap of 10 months between the time dispatchers were first trained to use the software and when it was actually implemented demonstrated its role in the disaster, the software had three primary flaws that caused an immediately failure: Imperfect data, interface issues and memory leak.
Robert Charette and risk management and IEEE member stated, “Bad decisions by project
managers are probably the single greatest cause of software failure today”. While the
software controlling the LAS’s CAD system had some key flaws, without which the system may
not have failed to the extent it did, the events surrounding the failure, the state of the LAS as an
organizational entity, and the process with which the LAS approached the development of the

Getting the LAS CAD system to the point of implementing was of a great challenge and it
opened the doors for failure. The originality of the LAS’s CAD
system was introduced in 1987. Due to the gross overspending by 300%, the project was
canceled in 1990 before being modified in 1989. Shortly after the national mandated to reduce
emergency response times pushed the LAS to look into a computerized
system once again. There were two people who were appointed in selecting a software vendor
for creating the system: a manager expecting to become redundant and a contractor who was a
temporary addition to the organization. These individuals’ roles and their lack of stake in the
project caused one to question their ability to select the best company for the job. Moreover, the
selection committee weighed a bid’s price as an essential factor in selecting a vendor.
Selection of a developing organization was further constrained by the requirement that the
project is finished within a year. Several companies proposed modified implementation dates
in which some functionality would be delivered after deadline and the rest a year later.
The LAS accepted a bid of less than a million that was done by a combination of
companies. The software portion of the system was “offered as a throw-in in a hardware deal”
for a meager 35 thousand and was completed by a company called System Options.
The majority of the cost for the package which relied heavily on software went instead toward
hardware should have raised a flag that something was wrong.
As the LAS pushed for expedient delivery, System Options in the area of software system
development had an obligation to protect the public. System Options took an enormous risk in implementing the project knowing that the system was incomplete and not tested. Even if System Options had a lot against the timely release of the software, the loss the failure caused was far greater than any monetary investment that could ever be made.
