Tadalafil( Cialis )

1. Generic name and brand name(s): The generic name for the drug is Tadalafil  and the brand names include Cialis and Adcirca. Tadalafil works to relax the muscles in the walls of the blood vessels, which increase the blood flow to different areas of the body. This drug is used to treat erectile dysfunction (impotence) as well as to treat the symptoms of benign prostatic hypertrophy (enlarged prostates).

2. Pharmacokinetics: When the dosage of Tadalafil is between 2.5mg to 20mg, the AUC increases proportionally with regards to the dose in subjects who are healthy. In this regard, we find that the steady-state plasma concentrations are achieved with five days if the dosage is given once per day. AUC then becomes about one and a half times greater than it is after a single dose. The tadalafil concentrations in healthy males tend to be with regards to if there are multiple doses of 5mg each. After a single dose is given orally, the maximum plasma concentration occurs between 30 minutes and 6 hours, with the median time being 2 hours. The mean apparent volume of the distribution is about 63 L, which means and shows that the drug is also distributed into the tissues. 94% of the tadalafil in the plasma gets bound to proteins at therapeutic concentrations. Tadalafil is metabolized to catechol metabolite using the CYP3A4. This then in turn results in the methylation and glucuronidation. The drug is excreted in the feces as well as the urine with the half-life of the drug being 17.5 hours.

3. Pharmacodynamics: It has to be noted that when tadalafil is administered in healthy males at a dosage of 20mg, it does not result in any significant differences in the blood pressure. No significant effects on the heart rate are there either. Other than that, we find that the tadalafil has certain effects on the cardiac function, exercise tolerance, as well as hemodynamics. When cardiac ischemia are enrolled in subjects who have taken the drug, it resulted in no clinically meaningful differences in the person. Other than that, it has been noted that while taking tadalafil, some blue/green color impairment have been noted in certain subjects. Moreover, there were no adverse effects that can be noted with regards to the uptake of the drug and the sperm count.

4. Indications: The drug is used for erectile dysfunction as well as for the signs and symptoms of benign prostatic hyperplasia.

5. Contraindications: It is important that any use of organic nitrates, even if taken intermittently, is something that is contraindicated with the use of tadalafil. Other than that, the drug is contraindicated in patients who have a known serious hypersensitivity to tadalafil. Several hypersensitivity reactions have been reported and these have included Stevens-Johnson syndrome as well as exfoliative dermatitis. It is also noted that tadalafil is not to be used with patients who are taking GC stimulators, such as riociguat.

6. Side effects: Tadalafil has been associated with several side effects. Some of the more

common side effects include headaches and dyspepsia. Other side effects also include upper respiratory tract infection, nasal congestion, cough, back pain, and myalgia. Most of these effects would be unwanted and it is important to consult a doctor if any of the side effects occur. The thing to note is that the side effects of tadalafil can be categorized with regards to their severity. With regards to major side effects, we find that some of the less common side effects include blurred vision, arm, back, or jaw pain, chest pain or discomfort, child, confusion, chest tightness or heaviness, cold sweats, dizziness, faintness or lightheadedness when getting up suddenly from a lying or sitting position, hearing loss, nervousness, shortness of breath, fast or irregular heartbeat, nausea, pounding in the ears, slow or fast heartbeat, vomiting, unusual tiredness or weakness, and sweating. Some severe but rare side effects include prolonged and painful erection of the penis. It has to be noted that tadalafil is also associated with many other side effects, many of them are rare and their incidences are not known. However, they are quite serious. These include blindness, cracks in the skin, diarrhea, double vision, blistering, peeling, or loosening of the skin, decrease or change in vision, difficulty with speaking, fast, irregular, pounding, or racing heartbeat or pulse, hives or welts, inability to speak, loss of heat from the body, red skin lesions, inability to move the arms, legs, or facial muscles, joint or muscle pain, numbness or tingling of the face, hands, or feet, red, swollen skin or eyes, scaly skin, slow speech, sudden cardiac death, and skin rash.

Other than that, some minor side effects of tadalafil have also been documented. These are the ones that would not usually require any medical attention. It has to be noted that some of these side effects are very common and mild and they would go away once the body starts to get adjusted to the drug. These side effects are usually nothing to be concerned about but they include such things as belching, indigestion, acid or sour stomach, heartburn, and stomach discomfort, upset, or pain. Other than that, some less common minor side effects include body aches or pain, congestion, dry mouth, excessive eye discharge, bloody nose, burning feeling in the chest or stomach, difficulty with moving, dryness or soreness of the throat, eye pain, difficulty with swallowing, feeling of constant movement of self or surroundings, hoarseness, fever, lack or loss of strength, muscle stiffness, loose stools, nasal congestion, pain or burning in the throat, vertigo, swollen joints, insomnia, tearing, and  voice change.

7. Adverse effects: Several adverse effects have been depicted with regards to tadalafil use. It has to be noted that some of the adverse effects are the side effects, such as dizziness, flushing, stuffy nose, muscle pain, back pain, upset stomach and headache. In patients who suffer from dizziness and lightheadedness, it is important to get up slowly with getting up from a sitting or lying position. The thing to note is that sexual activity is something that can place a lot of strain and stress on the heart. This is why patients who have a heart problem can have problems using this drug. If the patients start experiencing dizziness or pain in the left arm, jaw, or chest, it is important to immediately contact the doctors. In some cases, the use of the drug can lead to sudden problems with the vision, which can lead to permanent blindness in either one or both eyes. It has to be noted that if this occurs, it is important to discontinue the use of the drug and to consult a doctor immediately. Patients who have heart disease (including high blood pressure or cholesterol), diabetes, or other eye problems are at a higher risk of being afflicted with the blindness while using tadalafil. Moreover, if the patient is over 50 and is a smoker, it increases his chances of going blind while taking the drug.

Interestingly enough, some people might also suffer from hearing loss, which tends to follow or be followed by ringing in the ears. This is also something that can cause dizziness. This is why it these adverse effects occur, it is important for the person to stop taking the medication and to seek medical help. Some other adverse reactions to tadalafil include a serious allergic reaction, such as trouble breathing, rash, itching, and swelling.

8. Patient Variables:

a. Geriatrics: Tadalafil has been administered to patients who are over 65 years of age and this has shown about 25% higher AUC with regards to the clearance. It has to be noted that there has not been any need to adjust the dosage based on the patients’ age. The thing to note is that the elderly can take this medication; however, it is important for them to monitor their health while doing so.

b. Pediatrics: This drug has not been evaluated on people who are under the age of 18 years old.

c. Pregnancy: This drug is not indicated to be used in women and it has not been evaluated on the drug’s use on pregnant women.

d. Lactation: This drug is not indicated to be used in women and it has not been evaluated on the drug’s use on lactating women

9. Off label uses: Even though the drug is used to treat erectile dysfunction and the signs and symptoms of enlarged prostates, it has also been used for several off label uses. These include helping people climb mountains. This is because this drug allows the climbers to keep climbing without getting tired. They can also work normally in high-altitude and low-oxygen environments. In some cases, the drug has also been used to prevent jet lag. Tadafil is also sometimes used as a performance enhancing drug, as the dilation of the blood vessels results in the better delivery of the steroids to the various muscles and organs of the body. Tadafil is also sometimes used to treat muscular dystrophy.
