The aim of this essay is to demonstrate my knowledge and understanding of the care a nurse will provide for Queenie in the treatment and management of her leg ulcer.

Caring is best demonstrated by a nurse's ability to embody the core values of professional nursing. Core nursing values essential to baccalaureate education include human dignity, integrity, autonomy, altruism, and social justice. The caring professional nurse integrates these values in clinical practice.

Identify the chosen nurse and provide a brief rationale for your choice with supporting evidence.  Discuss the professional nursing values essential to care e.g. respect, confidentiality, compassion and dignity, the 6 Cs that you may expect of the nurse.

Queenie is a 79 years old female, a widow who lives alone in a two-bedroom bungalow. She is a mother, grandmother, and great-grandmother. Queenie has been very sociable, religious, independent and active woman in the community. She suffers from venous leg ulcer. A skin condition that happens as a result of poor blood circulation from the legs. The vein in the legs does not return blood to the heart and upper body which result in venous failure. (Venous skin ulcer-topic overview, 2015).

Treatment is carried out by a healthcare professional trained in compression therapy for leg ulcers. Usually, this will be a practice or district nurse. Choices, (2016).

A district nurse provides nursing assessments and cares for housebound patients with a physical healthcare need. They see patients at home and in residential care settings. (Central London community healthcare NHS trust, 2017).

Queenie is currently unable to carry out her normal daily activities because of her deteriorating state of health. Lately, it was found that one of her legs is now inflamed and infected, she is constantly in pain and mobility is hampered.

Delves Yates et al, (2015). The essence of nursing is delivering effective care to adults, children, families and whole communities. Holistic care method will be used to care for Queenie and her family.

The nurse will be addressing her physical, emotional, spiritual needs and analyse how it is affecting her general well being. (Perkins, 2012) this is to ensure no stone is left unturned

The district nurse will contact Queenie beforehand to agree with her on a visiting day and time which is convenient for her. This would indicate respecting her right, dignity and privacy. '''You must treat people as individuals and respect their dignity'''. NMC (2016).

In addition, consent should be obtained before proceeding with any nursing care. NMC (2016). The nurse needs to make sure they have the patient consent to carry out any care they want to perform and that the patient has the c

capacity to give it. Failure to do this will result in abuse of patients and has a legal implication. It is good practice to respect people'''s right to determine what happens to their bodies. Moreso, it is fundamentally a legal requirement.

Suggestion will be made to Queenie if she would like or that she ask her children, friends from the church to be present, because it gives the patient and her family the opportunity to be involved in decision making and creating a holistic care plan.

Assessment of the environment to ensure the safety of the patient and the nurse.  before proceed with checking her vital signs and other health assessment. Safety will be maintained by ensuring that the patient home environment is clear and free from any object or item that may be obstructing passageway and the space of a patient.

Queenie is having mobility issues due to the infected leg ulcer, the nurse checks with the patient if she is comfortable or in any pain and the severity of the pain.

Assessment will include checking patients baseline blood pressure, heart rate, and body mass index. The nurse will assess her wound, skin and lower limb vascular assessment. JCN Journal, (2013).

Leg ulcer can result in a great deal of disability, which can last for years if not managed appropriately (Moffatt et al,2009).

Demonstrate your knowledge and understanding of effective therapeutic communication between the nurse and the individual in the scenario; supported by relevant evidence.

Define therapeutic communication, identify any barriers within the scenario e.g. visual, hearing, body language, touch and describe the principles of effective communication to overcome them using relevant models of communication.

Queenie is at her most vulnerable state, it is important that the nurse is able to show care and compassion via therapeutic communication.

Communication is transmission of information either verbal or non-verbal, for example facial expression, talking and gestures. Nurses need to communicate effectively in order to identify if patient can understand what is been said (Holland et al 2008) The nurse check if there any language barriers (Non English speaker, in that case l will need an interpreter.

I will ask her if she has any worries, hope or concern since she is a widow if she is likely to be depressed also she lives alone and this can affect her mental status also depression can affect the ability of the patient to self-manage their condition. If she is experiencing this, l will refer her to a psychologist who will support in coping, Sally and Dallas (2005) furthermore patient diagnosed with should be empowered to self-manage their condition.

l will ask her about her sleeping pattern because sleeping enable body to relax from stress of everyday living. So it is important for individual to get a good sleep (Holland et al 2008) l will now educate her, on how she can position herself in bed, by sleeping upright and having more pillows to ensure chest expansion also

she is not visiting her friends, she does her house work which is demanding and it makes her to become breathless. This can be solved by a referral to social service to assess for a care package such as helping with AL

Eating and drinking is very important to maintain our body'''s homeostasis, we need to eat the right food and fluids to ensure correct balance diet. Roper et al (2000)

is report that she has lost appetite and thinks that she may have lost some weight. I need to find out her eating and drinking habits to ascertain that she is eating and drinking well, is she able to effort to buy and prepare food and drink. I need to find out if Rita is able to do so economically, physical and mentally, can she manage to do her own shopping buy the right food that will give her a balance diet? (Holland et al 2008).

I will need to find out if there is any cultural influence regarding eating and drinking also if she understands what a balanced diet is, what her feelings about eating and drinking, whether she is stressed about eating and drinking

She needs to be encouraged to rinse her mouth to avoid thrush. I also need to find out her eating like chewing, if she needs to adjust her meals to a soft diet or if needs a denture,

The question will be asked round her general well-being, how she has been managing to mobilise, nutrition
